110-300-0210(1)(2) Licensor reviewed child files and observed the CIS forms are not complete. 110-300-0210(3) Licensor reviewed child files and observed the children with conditional CIS forms did not have a signed and dated statement detailing when the child’s immunizations will be brought up to date and stating they understand their child will be excluded from care if the immunizations are not completed within 30 days of the specified due date.
